W-2 Error

It has come to our attention that there was an error in the printing of the W-2’s that were distributed earlier this week. The error does not effect the amount of taxable wages or the amounts withheld for federal or State tax. It relates to the information reporting of the pre-tax employee paid Wisconsin Retirement contributions. This amount was included in box 12 with a code G. The amount should have been shown in box 14. Due to the error, W-2’s will be reprinted and redistributed as soon as possible. We sincerely apologize for the error. If anyone has questions on this matter, feel free to contact Michelle at 832-6438.

Worker’s Compensation code change
Employees off on work comp will receive no pay from the City but will receive their payroll from the work comp carrier separately. In order to accommodate this, all employees (except Police, Fire & VT unions) will have to use the new “RW” code. This stands for report work comp no pay. Therefore, your employee with this code will have their regular hours for that pay period reduced by the RW code hours and only should receive pay for their actual hours worked. The WC code has been deactivated and you will not be able to use it by accident. If you have any WC questions, please contact Jay or Kim directly.

Holiday Break Activities & Winter Recreation with Appleton Parks & Recreation - have fun & get fit over the holiday break! Take advantage of the great outdoors and the numerous winter recreational opportunities we have to offer right in your backyard! Spend an afternoon or evening with your family at one of our four winter warming shelters or at one of these other great activities like open swim at West Pool, Zumba downtown in City Center, open gym at Ferber or free open skate at AFIC. Ice rinks and warming shelters are scheduled to open on December 26, weather permitting. Winter Water Carnival is on December 28 - ages 6-13 with fun aquatic activities & games. Daily admission applies.

Adopt-a-Family

This year for 2011 the Library has once again adopted a family for the holiday season through the Salvation Army. Everyone on staff is invited to participate, however participation is completely optional. Our family has a single mom and three boys ages 17, 12, and 8. Like past years, the children have wish lists, however unlike past years the boys have asked for items that exceed the limit we are supposed to spend on them ($50) such as bikes, and I do not expect staff to provide three bicycles for the family. They have also asked for cold weather items such as hats, gloves, and boots. If you wish to purchase one of these cold weather items let me know and I will give you the child’s sizes. I have some sizes for shirts and pants as well, but not for all three children. The mother in the family has also asked for kitchen items, which you may wish to donate. Items should be placed next to my desk or on my chair. You do not need to stick to the wish list, if you choose you may purchase items like candy, DVDs, favorite books, gift cards for music. You may also donate money by cash or check which will be used to buy a grocery gift card to Woodman’s or additional items on the wish lists. I do my best to ensure that each child has the same number of gifts.

Any monetary amount (as little as a dollar!) would be welcomed.

A few guidelines:
· Please do not wrap the gifts.
· You may provide gift wrap.
· We are asked not to spend more than $50.00 total on any child.
· The family will receive a grocery store gift card.
· Gifts must be turned in to Ellen Jepson by Tuesday December 13th by noon.
· Please bring any items you have purchased to Ellen and place them next to my desk or on my chair.
· You may donate cash or write a check (payable to Ellen Jepson) which will be used to purchase additional items.
· Any questions, speak to Ellen in the Children’s Section!

As in years past, the flu vaccine is available at no cost to all elected officials, employees, spouses and their children (children must be at least 9 years of age or older).

ETF Group Appointment Session Locally

The Wisconsin Department of Employee Trust Funds is offering retirement Group Appointment sessions in the Appleton, Oshkosh, and Fond du Lac areas. I am writing to you to ask if you would forward this information to your staff. The next available appointments are on Wednesday, December 14 at 1:30PM or Thursday, December 15 at 10:00AM. To attend one of the sessions, a Wisconsin Retirement System (WRS) participant is required to have both an appointment and their retirement packet in hand upon arrival to the session. To sign up for an appointment or request a retirement estimate, one can do so by calling the ETF call center at 1-877-533-5020. (IMPORTANT: Getting a retirement packet from ETF does NOT obligate one to retire in the WRS. The participant simply has their application and packet in case they do decide to retire sometime over the coming year.)

Any participants who are within one year of their minimum retirement age in the WRS (age 50 for protectives, age 55 for non-protectives) are welcome to contact our Department at 1-877-533-5020 to request a retirement packet.

ETF has tailored Group Appointments for members planning (or considering) to retirement. Topics covered include the retirement annuity (pension), health insurance, life insurance, return-to-work rules, etc. The Group Appointments offer a participant the opportunity to ask questions of knowledgeable, experienced ETF staff.

From Thursday September 29th to Monday, October 24th the Appleton Public Library conducted an online survey to collect information about how our patrons use the library’s computers and Internet connection and how this service makes a difference in their lives. This information will help the library evaluate our current services and to communicate the value of public access computing. The IMPACT Survey collects data across the following areas: general use of library and online resources, civic engagement, eBusiness, eCommerce, Education, eGovernment, employment, health & wellness, and social inclusion.

The results of the survey as well as how patrons used library and internet connections for any other purposes and suggestions for how computer and internet connections could be improved can be found on the "other links" column on the intranet.

Through the survey website, our library also gains access to new tools to help us communicate the value of public access technology.

Our patrons will access the online survey through buttons on our public access computer workstations, the library website and paper copies of the survey will be available throughout the Library. If you need a paper copy, here is the link. Patrons must be 14 years or older to complete the survey. The survey takes 10-15 minutes to complete, is completely anonymous, and is available in English and Spanish.

The closing concert of the 2011 summer Appleton City Band concert season is scheduled for Tuesday, August 16, 7:00 p.m. at Pierce Park, rain or shine. The Appleton City Band is directed by Mr. Ike Spangenberg. This will also be Mr. Spangenberg's final concert as he is retiring as conductor after 45 years of providing people with the sounds of music. Cake and ice cream will be served following the concert. UHC Pharmacy Benefits

As you are aware, UHC proactively manages our pharmacy benefit. As part of this management, UHC has benefit updates taking effect on July 1, 2011. Please see attached for detailed information of up tiering, down tiering and new exclusions, etc. These updates represent a small percentage of all prescriptions – no medications in the top 50. Among these updates are some medications that will be excluded from benefit coverage. A majority of these medication exclusions have minimal use so few employees will be impacted. UHC realizes any impact may be difficult, so they only make the decision to exclude based on clinical evidence and when equally effective alternatives are available.

2011 Butterfly Festival

UnitedHealthcare launched a Preventive Care web-based tool (www.uhcpreventivecare.com) that provides age- and gender-specific preventive-care recommendations that enable people to better manage their health. Users can set e-mail reminders for any of their recommended preventive screenings or immunizations and, working with their doctors, use this knowledge to make informed medical decisions. They can view, print and e-mail specific preventive-health guidelines and access useful tips, tools, resources and materials to support their overall health. The tool is completely mobile-enabled for use on Blackberry, iPhone and iPhone devices, among others. This is a wonderful resource for anyone to use, regardless if they have coverage through UHC or not.

If you are a City employee and are eligible for the City health insurance program, you and your spouse can participate in this FREE screening and counseling session.* The enrollment deadline date is March 4, 2010 by 4:30 p.m.

Payroll Notice

Due to the recent signing of a tax relief bill by the President, you will notice an increase in your net pay this pay period. This is due to the 2% decrease of social security taxes withheld. This decrease will be in effect for the tax year of 2011.

The 2011 Federal tax tables have been received and will go into effect with the second payroll of January. With these new tax tables, you will see an increase in the Federal taxes withheld.

"New" home page

Forgive this long posting...

We will be going live soon with some modifications to the home page. This page doesn't make dramatic changes to the existing site, and has nearly identical content to the existing home page. What has changed is how and where we are displaying information.

This page will be less text-oriented and try to tell more stories with graphics. It will contain a slideshow that features a mix of upcoming important events, and some of our "core" services we want to promote. For example we will have core slides for:

Friends
Volunteers
FB page
WIFI
Databases
Recurring program series
Etc...

These core slides will rotate in and out as we have space in the slideshow or as they are seasonally relevant.

Below the slideshow you will notice blurbs similar to what you see on "what's new" currently. We will be referring to these as "stories." The slideshow and stories are in the center of the page to give them the most prominent positioning. The stories are fed through a blogger blog that staff will be asked to contribute to. The things that go in the blogger blog are decided by a subset of the marketing committee that Michael has assembled.

If you have ideas for slides or stories please let Michael Kenney know. He has a calendar of upcoming programs which helps us find places where we need to focus on specific important events (for example, the book festival, SLP, etc...) - this calendar also gives us the ability to see where we may have some "down time" in the calendar to rotate in more of the core slides.

The left column is very similar to what you are used to with the existing page.

The right column features a feed from the calendar with brief listings. Visitors to the site can click through to the more complete entries.

Please take some time to get familiar with the page before we go live with it. We would like to put it out for the public in the next week or so.

Sara has done an amazing job on this and I hope you will enjoy it.

Twelfth Annual Give a Child a Book Campaign Now Underway

Individuals, families, businesses and organizations are invited to donate new books to the 12th Annual Give a Child a Book Campaign. This book drive is being conducted by the Friends of Appleton Public Library. The books are given as holiday gifts to children from low-income, at-risk homes and other area groups in the Fox Valley by the Salvation Army, just before Christmas.

Vacation Payout Into PEHP Request Form

Per policy, up to two weeks of vacation remaining at year’s end is automatically carried over to the next year, and additional vacation is converted to the post-employment health plan (PEHP). If employees wish to convert any of the first two weeks of vacation to PEHP, they must fill out this form and submit it to the Library Director for approval. Approved requests from the department director must be forwarded to Human Resources by no later than December 15.

Although some of the PEHP conversion is automatic, it still requires proper entry on your year-end timesheet. The form provides instructions – but Library staff should note that where the form says “enter in payroll”, we mean “write on your timesheet.”

The Library Board President introduces the next Library Director

Good Morning!

I realize that what I am about to announce is not exactly brand-new information and will not come as a surprise to you. However, both Terry Bergen -- who has been on the Board for many years and is our current Personnel Committee Chairperson -- and I felt strongly that even though we had hired an internal candidate, it was important to follow many of the same formalities in making this presentation as we would if we had brought in a new director from another city.

Following Terry Dawson's public announcement of his retirement late this summer, I met with Terry Bergen and we appointed a Search Committee to find a new Director. That committee consisted of:

the Board's Personnel Committee

Terry Bergen
John Peterson
Jean Detjen, as well as

Jan Quinlan, the Executive Director of the Friends
Peter Gilbert, who is on the FOAL Board and is the Director of the Lawrence University Library
Rick Krumwiede, the Director of OWLS
Don Hietpas, who serves on the Friends Board and on the Library Board
I came along for the ride -- but was not a voting member -- and we also had the benefit of the expertise of
Jay Ratchman from the City of Appleton HR office

We posted this position nationally, and received applications from every corner of the country and many states in between. Even though we have a very capable staff here, we truly did not know whether anyone from the APL would apply for the directors post.

The Search Committee spent a great deal of time deciding what qualities and qualifications we were looking for in our next director. You might recall in mid-August receiving a request for input from Terry Bergen and me regarding your ideas for the new director. I would very briefly sum up that input like this:

"Please hire a director who understands the importance of each section of the library and will not be biased toward or against any section. Oh, and hire someone who has Vision."

Committee members reviewed stacks of application packets and narrowed down many impressive candidates to only three finalists we felt had the potential vision and breadth of experience we were looking for.

In late October, the Search Committee interviewed the finalists. The Mayor and Sandy Neisen from HR had a chance to meet each candidate and APL section heads conducted tours with the two out-of-town finalists. Each of these groups provided feedback, and our new director was selected.

The twelfth director of the Appleton Public Library, assuming those duties after Terry's retirement in January, will be Colleen Rortvedt!

As you may know from working with Colleen, or have read in the recent Post-Crescent article, she graduated form Lawrence University and earned her Masters in Library Science from UW-Milwaukee, while working here at the APL. Colleen has that wide range of library experience we all felt important, having worked in Young Adult and Audio-Visual, coordinating the development of the Hmong Collection, and planning the first community read here, all prior to becoming the Assistant Director nearly two years ago.

The search committee felt that Colleen had an appreciation for the many different types of people within the Appleton community and their needs, as well as an understanding of our role as the systems resource library and a vision for advancing the APL into the future, considering the physical, technical, financial, and political challenges it will face.

We know that after Terry's retirement, this library will be in very good hands, but things will not be exactly the same, and it would be really weird for us to expect them to be, and so I would ask everyone to be open to the vision and new and different leadership that Colleen brings when takes the helm in the beginning of the year!

2011 Medical/Dental Insurance & Flex Spending Enrollment

You will have the ability to make changes to your election as many times as necessary between now and December 2nd at 4:30 PM. This system will only work if you are signed into the computer as yourself. Please verify that your own username appears in the upper right hand of the computer screen. This ensures that the election is being made on your login account. If you are on someone else's computer, you must log them off and sign in under your own account in order for the election process to work for you.

2011 Prescription Drug Changes With UHC

Changes to tier structure can make an impact on your flex spending election for the new year.

This new program through UHC targets a select number of high-cost drugs while still providing affordable choices. This program offers members three cost-savings options. First switch to mail order pharmacy and keep current medication. Second, switch to a lower-tier medication and keep getting at your local pharmacy. Third, switch to lower-tier medication and go through mail order pharmacy and save even more money. The following targeted medication and products are affected by this program.

Benign Prostatic Hyperplasia (BPH) – Avodart, Uroxatral
Depression – Lexapro

Appleton Public Library announces new Library Director

APPLETON, WI – The Appleton Public Library announced today that Colleen Rortvedt has accepted the position of Director at the Library, effective Jan. 8, 2011.

Terry Bergen, Search Committee chairperson noted that the applicants from the extensive search brought in responses from a large pool of candidates.

“We were gratified at the high caliber of applicants for this important position,” said Bergen. “The search committee had a difficult decision, but following an intensive interview process our choice became clear. We were particularly impressed with Ms. Rortvedt’s leadership and vision,” added Bergen.

Liz Truesdale-Witek, President of the Library’s Board of Trustees as well as a member of the Search Committee stated, “This was a national search - we received applications from California to Vermont and many states in between. Colleen Rortvedt not only has the breadth and depth of experience that is required of a director the size and caliber of the Appleton Public Library, but she also appreciates APL’s special role as the resource library for the Outagamie Waupaca Library System. She demonstrated an unparalleled familiarity with and concern for the needs of the diverse community the Library serves, as well as an in-depth understanding of the other challenges unique to the Appleton Public Library, and set out strategies for addressing them.”

The search committee included the Library Board Personnel Chair Terry Bergen, Board President Liz Truesdale-Witek, Board Vice President Don Hietpas, Attorney John Peterson, APL Board Trustee Jean Detjen, Outagamie Waupaca Library System Director Rick Krumwiede, Friends of Appleton Public Library President Peter Gilbert, Friends Executive Director Janice Quinlan, and Jay Ratchman City of Appleton Human Resources department.

“Libraries are about more than books and materials – we provide opportunities, bring people together and help solve community problems,” stated Rortvedt. “I’m looking forward to continuing APL’s mission in serving this wonderful community and I am honored to be chosen as the next Library Director.”

Rortvedt, a graduate of Lawrence University, received her Master’s degree in Library Science from the University of Wisconsin-Milwaukee. Ms. Rortvedt has worked at other libraries prior to joining the Appleton Public Library staff in 1996. She has served in several capacities at the Appleton Public Library, including APL’s first Young Adult Librarian, Audiovisual Librarian, and most recently as the Assistant Library Director. Her accomplishments include planning the first area community read “Appleton Speaks”, as well as coordinating the development of the library’s Hmong Resource Center.

“I’m pleased that the Search Committee and Board have chosen Colleen, who has already demonstrated her vision and strong community connections,” said Library Director Terry Dawson. “She has been an effective leader and manager with a track record of developing collaborations to improve library service and strengthen our community.”

Rortvedt will succeed Dawson, who is retiring after 32 years serving the Appleton Public Library, on January 7, 2011.

The next monthly meeting that Drew Habermann from United Health Care will be on Thursday, October 21 from 12:30 - 2:30 at City Hall in the Human Resources conference room. These monthly meetings are for employees and/or your spouses to meet with our medical insurance representative regarding any issues/questions you may have regarding your UHC policy.

Microsoft Office 2007 Home Use Program Available

As part of a new offering from Microsoft, Technology Services has arranged for you to participate in the Home Use Program (HUP). This enables you to order a licensed copy of Microsoft Office 2007 Enterprise to install and use on your home computer as long as you have a valid City of Appleton email address, which Microsoft uses to verify your City employment. You only pay the shipping/handling costs of around $20.00. Normally this product retails for well over $400.00. The main programs in this suite include Microsoft Word, Excel, PowerPoint, Publisher, Outlook and Access.

You may order a single copy of Microsoft Office 2007 to install and use on a home computer. and you can continue using this HUP software while you are employed by the City of Appleton. While this is offered through the City's contract with Microsoft, please be aware that the TS Help Desk cannot provide any support for installation or use of this on your home computers. You can contact Microsoft directly for that support if needed.

To access the Microsoft Home Use Program web site, please follow the steps below:

1. Go to https://hup.microsoft.com/
2. Select the country to which you wish your order to be shipped to and choose the language for viewing the order web site.

3. Enter your corporate email address (must end in appleton.org or apl.org) and insert the following program code FC89D918AD . Note: this program code is assigned to our organization for our sole use in accessing this site. You may not share this number with anyone outside our organization.

4. You will receive an email from Microsoft containing the link to the actual order site. Please read the Program Guidelines before attempting to place your order. Place your order on-line and it will be shipped to the location you have chosen. Please note that a fulfillment fee will be charged to cover packaging, shipping and handling costs.
