The Pharmaceutical team at UHC has made their mid year changes to the prescription tiers.
Drugs going down a tier level are: divalproex sodum ER, Somatuline Depot, Nutropin AQ NuSpin, acarbose, clindamycin, ketorolac tromethamine, nateglinide and Opana ER.
Drugs going UP a tier level are: Aciphex, Dipentum, Protonix, Tazorac cream and gel, Vytorin and Zegerid.
For more detailed information, please read the attached document. If your drug is going up a tier, you will need to discuss alternatives with your physician in order to find a drug at a lower tier copay cost to you and get a new prescription.
